SB 12 Parental Consent Forms: Routine Health and Wellness Services
- Starting September 1, 2025, in compliance with the new state law (SB12), Texas public school districts are now required to receive parental consent before providing even simple or routine physical and health services to students. This does not change Melissa ISD’s commitment to our students, but it does require parents to provide written consent in order for their students to receive school-related physical and mental health services. Please read and complete the Consent Form. For more information, visit the Melissa ISD Routine Health Services Consent website.
Secondary Parental Consent Form for Library Book Checkout (Grades 6-12)
